In Sharepoint 2007 there are some ways to determine the user's login or name which should help you. I use an Active Directory web service which identifies the form user by their network login, even if they are accessing remotely via VPN, etc.  This can also access any data in Active Directory such as location or manager, etc

Well the one issue is the 4000 records.  You should probably look at switching to web services so you can just use a data connection of 4000 for the first drop down field. then query the specific record in the drop down so that you are not sorting through 4000 records in the form.
