We had a corporate GPO security setting called Custom Code which was disabled. It can also be accessed through a reg key (HKCU\Software\Policies\Microsoft\Office\12.0\InfoPath\Designer\RestrictedFeatures\CodeAllowed). Change the GPO setting (or remove the key) and all works fine.
