Oh. I guess it never occurred to me to ask how you were submitting - I'd assumed you meant in the re-opened form.
I'd probably add a section meant to be displayed while emailing that had the form info in expression boxes or something instead. Controls don't always format nicely in email.

Yes, it is the date picker. This is the image I receive in my Outlook email.

The control in the second image doesn't look like a date picker. Is it the same control? Or is it open in IPFS (can't remember off hand if the calendar shows always in IPFS).

So - I originally put in a formula that would set the value of the last instance of the row.

I should have stated to replace the placeholders here as well:

with the actual URL to your SharePoint site where the list is. Did you do that?

I found my GUID, e74ed205-13c1-45ae-8198-20890d6aef50, placed it in the URL you provided, and used that URL in my address bar. The result was an error stating IE couldn't display the page. Sorry, perhaps I misunderstood what I should do with the URL.

Just to update you,
I have been playing around with the font formatting.
If i change the font style to "Heading 1" and then reduce the size down to 10, black etc - it displays in browser as I want.
However
