 Your submitting your form to  2 different locations  -  Workflow would run at the item level of the List you set it to.  I did check MSDN and the Picture Box Control is compatible for Forms Server 2010, ( Ive Tested this and it works) Just weird they disable the cut and paste functionality(rendering in I.E.) when it works in preview(Client opens)!!  yoursite/Lists/Workflow History    is the default associated history list you can see the history and outcome more detailed info And check each list setting if its firing off more than 1 list or ? more than 1 item is being created.
