I am pretty sure we the version of the mapping form you have won't work with IP 2013. We do have some additional IP 2013 fixes in newer versions of qRules. I'm afraid you may need to upgrade - I'd offer to just get you a 2013 compatible version of the mapping form, but I'm pretty sure we had other changes to the form / ...

You can use ChangeConnectionUrl with a SharePoint list connection then. :)
The additional undocumented parameter you need is listguid. This needs to be the GUID for the list you want to query.
You can get that with the lists.asmx web service: http://msdn.microsoft.com/en-us/library/lists.lists.getlist(v=office.12).aspx

Yes Full Trust security level is set for both sites and I upload template using Central Administration.
