You can place just the one radio button you want to hide and its text in its own section control and then use conditional formatting to hide it using any condition you choose.

You could use conditional formatting to only display the hyperlink when it's supposed to be clicked.

The sample uses a hyperlink in the form, which the user can click to open the other form.  As you've discovered by now, rules with "Open a new form to fill out" don't work with dynamic URLs so that will not work.  Is a hyperlink not an acceptable approach?

Are you talking about debugging code, or debugging the non-code workings of your form? For code, you can start up your form in debug mode, set breakpoints and observe the values of variables at various points. For non-code stuff, there isn't really any straightforward approach.  You can temporarily create expression boxes, etc. to ...

I'm confused by what you mean by this: The Insert Item text is working as it should be, but the fields have disappered from the table row. You can still add more rows, but they're all blank. Aren't new new rows in a repeating table typically blank?  Do you mean the values in the rows can't be edited?  I'm not 100% ...
