InfoPath Dev

I have published an InfoPath Form to a SharePoint library and it has worked with no issues until today.  All of a sudden, when I click the New menu and choose New Document to open the form, it gives me a blank page with message "The Form Has Been Closed"  I also notice upon clicking New the URL is  the following:  http://sharepoint.companyname.com/sites/homesite/Training/_layouts/FormServer.aspx?XsnLocation=http://sharepoint.companyname.com/sites/SAG_BWT_BCC/Training/Mailbox%20Communications%20Response/Forms/template.xsn&SaveLocation=http%3A%2F%2Fsharepoint%2Ecompanyname%2Ecom%2Fsites%2FSAG%5FBWT%5FBCC%2FTraining%2FMailbox%20Communications%20Response&Source=http%3A%2F%2Fsharepoint%2Ecompanyname%2Ecom%2Fsites%2FSAG%5FBWT%5FBCC%2FTraining%2FMailbox%2520Communications%2520Response%2FForms%2FAllItems%2Easpx&DefaultItemOpen=1

 If I publish the form to a new library, it works with no problem.  Any ideas what is causing this?

No changes to the library or the template, correct?  I've had this happen before when closing a form, then accessing it again with the same url, so it may be an issue with one of the parameters.  But to get this from the new button is kind of strange.  What happens if you refresh the page once on the "The Form Has Been Closed" page?

Right, there have been no changes to the library or the template.  I have even republished it but the link for New Document still seems corrupt.  If I refresh nothing happens.  The blog article didn't help much.  If I cannot find a resolution I will have to recreate the library from scratch.
