I want to create a Master form that, based on one's selection would call the appropriate form (see below).  Is something like this possible?  I greatly appreciate your time and help.

An action on a button can be to open a new form.

Do you want a new document or are you trying to link to an exisitng document?  An existing document is trickier as you have to have a way to look it up.

Browser based forms you can use hyperlinks.

What is the environment are you using Sharepoint?
