The form is configured to fetch info from getuserprofilebyname on load.   But I test it after as admin I can chang usernames and the same issue is appearing.   We are using AD user's automatically implemented into sharepoint is it possible to recreate it in this case ? Thank you 

Thank you,   I got this report in attachment txt   Any Idea  ?   It says that it cannot found user. But after click OK,  I see it found user. However each time user recieve an error on connection. I have same issue with another user who is  old in our env.
