In the promotion process, spot check your columns being promoted and make sure they all are set to use existing ...

When publishing the forms (do NOT use Quick Publish in IP 2010 if you are having this problem), when you get to the column promotion list, open your columns and make sure the columns are using "existing column" (or something like that) instead of "create new ...
