The issue is that the fields you added are not actually present in the current XML. The XML have to be opened with the new template for the new fields to be added via the versioning behavior.

You could also copy your view into another view - and mark the view read only - then switch to that view if the field = submitted in a form load rule.

Hi - Wouldn't you just add some data connections to your SharePoint lists and then display the secondary data as desired on your form? You could add a button that re-queries the secondary data sources for them to use to refresh the data.

I guess if it were my form, I'd trouble shoot by showing the ribbon and trying the print preview button. If it switches to the print view there, I'm not sure why your link wouldn't work for it. But my thought is it won't - so from there it is a matter of trying to figure out why. You could try a different view and a different print ...

Each XML has to be opened with the new template for the new field to be added - and under form options, check your versioning settings to make sure you have the form template set to upgrade existing XML or they will not have the field added when they are opened.

In your form load rules you could have a rule with a condition that if the date field or the drop down field are not blank, set the checkbox to false.
