Wow - I've not tried that scenario (and I don't think it was one we tested for). I'll check it out in the next day or so and get back to you on it.

If it is not a repeating field, you could probably do this with CAML, no code needed - http://www.infopathdev.com/blogs/matt/archive/2006/02/02/Add-SharePoint-List-Items-with-InfoPath.aspx might help you.

Well, it is a compiled DLL in the form - you need the code source files. Do you know who originally developed the form? That's where I'd start.
-
Well, to directly answer your first question - no, you need code to know what view the form is on.
I am puzzled by the print preview behavior you are describing - opening print preview is re-executing your form load rules? Or is it that the Read Only view doesn't have a print view? Or perhaps something else is preventing the print view from ...

I'd use a concat formula and preceding-sibling. Sample attached, save it locally, right click and select design to open in design mode.

What is setting the date fields? Are there any rules setting them? Any default values?

Oh, InfoPath makes it SOOOOOOO easy to accidentally add custom code - you could have accidentally just clicked the wrong thing.
So you know what to do if it happens again - Go to the Developer tab on the Ribbon and click the Language button. Then click the Remove Code button. Save & close and re-inject with qRules and you are back in business.
-
I removed them both - have a great weekend!

It had never occurred to me - but your form has code - I can see a compiled DLL. That means you'll need the source code for the form to figure out what it's doing or how to replicate it.

Looks like you may have accidentally added code to your form. Here's a copy with the code removed - reinject with qRules again and give it a try.
