I do not believe this is a real issue. I believe this is how you are naming the files. If the file name changes then it is going to add a new file rather than replacing the original. What is your naming scheme? What is the original file name? What is the new file name?

You cannot disable it, that is why using your own button and not the toolbar button is preferrable. I assume you are referring to the button in the toolbar as the "built in".
