Can you explain exactly what you mean by workflow form as it pertains to InfoPath?  I know how to do a lot of things related to this topic, but I'm not actually sure what you're asking.

I don't understand the question.  if you want to get data from a SharePoint list or library, then you create a data connection, which will pull all the data from a given list or a library.  Very simple without any code.
