Hello, You can publish three times with different names using one form. For this you don’t have any problem with schema / ID. Do you have any problem of doing this?

Hello John, For hiding table also we have to add one section and in this section we can have our table. Then we can hide this section as per our requirement

Thank you swathip , In my case the Cascading Drop down box contains 10  items,when open form in DD box the first item is blank.like <blank> India Pakistan Russia..... In list box also contains the same problem.  How i remove this <blank>Itms in both the cases.
