Welcome Stefan.
I think you would want just one form but with two views within it. One for the search and one for the lookup detail result.
I would suggest you look at using web services to interact with your SQL data, if this will be an enterprise solution.
If you want to send me an email I can show you a couple examples of similar forms.

Hi Dean.
Why don't you send me an email and we can discuss what your requirements are.
We have several healthcare facilities that use our product in a similar way to record patient visit information. Some use it in conjunction with a paper chart and others with a completely electronic chart.
What you are describing is doable and we can run ...

Probably you would set up a data connection for each unique email destination.
Then create you submit button and use rules.
Add a condition to check the dropdown field value and if 1 submit to Data connection 1 and so on.

Hi Dean.
Here is the fix for that. DBXL Installation Produces an HTTP Error 405 on Windows XP Machine
Also make sure you are installing on a PC that is joined to a domain.

Hey Dave.
Does it pass design checker?
Does it say it has published successfully?
I guess with an admin form you might not get the same validation but I am guessing it is something in what your adding. Have you tried making copies of the views that you know work. I have had issue with timeouts buts not that blocked the form.

Two ways to do this.
The easiest is create 10 rows in your form but hide 9 of them with conditional formatting by default. Then based on selection unhide them.
Another method would require code to loop through and insert the number of rows to the user selects. If you 10 is your max then I think the former would be easier unless you ...

I think you are missing the submit data connection which is where you specify the location for the XML to be saved. Then you select this data connection for your submit action.
Tools -> Data Connections Go through the wizard.

I would do a couple things. First would be to leverage Windows offline folder and synchronization features.
I would use a network location, that everyone can access. Then have them create off-line access to it.
This will accomplish your sync and allow your path to remain the same for everyone.
Windows offline folders work this way.
So ...

First add your button.
You would need to go to Tools and Data Connections and create your submit if this has not already been done.
Then set the button type for Rules
Go into Rules and add a rule with an action to submit and pick the approriate data connection.
With Rules you can set values and other logic as well as have multiple submits.
I ...

You would want to use the Insertfunction and select concat first.
concat(RANK, ",", LASTNAME, "-", STARTDATE) or maybe you do not want the seperating characters but that would be the syntax.
I would probably add a field to your form. Add a rule on submit that sets the field to this formula if it is blank. That way it gets set ...
