Unfortunately, Sharepoint does not write back to AD, and AD will not write permissions to Sharepoint unless the AD group is a member of a current Sharepoint group. If you want to sync the AD and Sharepoint permissions, add the AD group to Sharepoint. You will not be able to see who is a member of that group from Sharepoint, ...

When you are viewing the library, what is the icon displayed next to your submitted form? Is it an InfoPath or XML icon? Just wondering how the document library is seeing your form. Also, check your file associations to see what your Infopath files are set to open with. Let us know what you find.

This sounds like it could be a firewall issue. Do you have a firewall running? Also, make sure you have added your SharePoint site to your list of trusted sites. Not doing that can create all sorts of problems. Let us know if that works.

Once verify is there any condition set on the rule to execute that only for first time. Also verify that the type of the value which is setting via rule for a saved form. Is that matching the type of the field or not?
