What I did was to enable version control in Central Administration and on the Forms Server, but it was just the latter that I was supposed to do. To update the form I now upload a new document in the Form Templates folder. Uploading it through Central Administration was only necessary for the initial release.

Easiest way would be to use User Roles and have conditional formatting based on the user role that hides or shows a section which contains the signature block. If you need to sign the entire form, then you could have two views. One which has the signature, and another which doesn't and switch the views on Open ...

Well, maybe this is an encoding issue. Open the XML of the form in Notepad and tell us what you see in the top of the file... Do you have any programming code (i.e. business logic) in your form?
