Filler or Browser form? If browser, check the SharePoint logs. I'm sure you've already verified permissions on the form library? Does this happen when updating existing forms, submitting new forms or both?

When you select the field for your count function, in this case Status, you added a filter for Status = Active. Modify the filter to include Type = Risk. Attach your form to a reply under the Options tab if you need a hand. :)

I want to count the number of instances where the Status is ACTIVE and the Type is RISK. I have count(Status[.="ACTIVE]) which works fine but how do I add the second criteria Type = "RIsk"?
