So - when you preview with existing data and your template is set to automatically upgrade the XML, your new field is still not included?
-
As long as you don't overwrite the template in DBXL it won't change it.
-
Your update was the clue to the issue. Under Form Options > Versioning, the template is set to Do Nothing, so new fields are not being added when you open an existing XML.

My guess is that for some reason the field that the checkbox is bound to is not included by default in the XML. You can check on the defaults under Tools > Default Values (I think, sorry, no IP 2007 around) or just attach under the Options tab if you want me to take a look.

Sorry, this is not something InfoPath does out of box nicely. You may want to consider your options for submitting your repeating data to its own list. You can do that with custom code (or some clever CAML) or qRules can make it easy for you - free trial here.

It really shouldn't take 10 minutes to load the form. Is that the only data connection? Is there other logic that may be going awry? Can you create a test form that just has a data connection to that same list and see if it also takes 10 minutes to load in the browser?

The things I know that turned out to be the most important to me when working with InfoPath are:You can have multiple controls bound to the same field, even in the same view (you know this one now ...

One thing that sometimes is confusing about InfoPath is that you can have multiple controls bound to the same field. Just go to your data source task pane, find the repeating field that is bound to your multi-select list box, and drag it onto your form - it will probably give you a selection of controls to choose from - you could just go ...
