I think I may have found the problem
It seems as though the form is not being resumitted back to the library
Is it possible to have to submit options on a single form. When I change the options on the reject button, the submit options on the submit button also change.
How do I go about creating a simple form with reject and submit ...

It seems you have attached a certificate with the form, removing the certificate from the template or installing the certificate on another PC also will help you.

well your submit button is doing exactly what it is supposed to do. Whenever you submit the form it should create a new form.
you store your form name in some variable and then submit the form (using that variable as file name) that should solve your problem

by deploy i meant publishing to the site, when you publish a form to a site and then the first form takes lot of time after which the performance is ok. But your seems to be a different case.

Make sure that your text box is set to be "Multi-line" in its properties too.
