I don't have access to the servers unfortunately. Does something need to be enabled at the server level in order for it to work? I would have to work with the SP Farm support personnel.

The URL should still the same. Check the server's ULS logs, that may give you some insight into the issue.

Hilary, I'm having this same issue. Was this ever resolved? It worked fine with SP 2010, but now I'm moving to SP 2013. Is the URL the same? _vti_bin/lists.asmx?WSDL

Is the form published as a content type? Or directly to the library? Do you publish to different sites, like a DEV / TEST / PROD scenario? Or did you just publish back to the same site?
