Hey - do you have this issue when you open existing files with the new template? Or also when you fill out a new form?

When I create a database bound form, by default I'm getting a repeating group for the data fields in the data source - is that not happening for you?
-
Hi - you can walk through the data connection wizard for the data connections to change whether something runs on load - it is in the last dialog in the wizard.
I think for your filter you want this instead:
d:Department_x0020_Code = xdXDocument:GetDOM("CAS Department Numbers and Names and ...

So, when you look at the promoted properties in the form, you can rename the promoted field (so that the name in SharePoint is different from the actual field name). Also, if a form library has multiple content types associated with it, you may have columns from a different content type....

So - I'm confused. :) Are you saying there are columns in your SharePoint library that you think are promoted from the form, but do not show in the promoted properties for the form?

You can find this under Form Options --> Promoted Properties
IP 2007: Tools > Form Options
IP 2010 & 2013: File page, Info, Form Options

Which suggests creating a web app with classic authentication - not sure if this an option for you, but thought I'd toss it out there....

This sample has the expiring version of qRules injected into it and may need to be re-injected to work correctly.
The sample form demonstrates using DateAdd in a repeating group, and includes a Holidays XML to demonstrate exclusion of weekends / holidays.

Are you using the form in the browser or the filler? See the user name in the Unauthorized? It looks like your ShP 2013 site is now claims based authentication (User: 0#.w|MyDomain\ofer.carmeli - I think this is the default now) and I'm wondering if that is what is causing the issue.

Thanks for the screenshots. In general, a lookup column will store the ID of the item in the list we are looking up on. So, that is the first thing I'd make sure of in the form - that in the drop downs for the lookups, the ID field is the Value in the drop down properties.
