what exactly do you mean by centrally located in sharepoint.
just give permissions to everyone to read that list

issue tracker web part stores the data in some list, track that list (open the page that contains issue tracker and try to edit the webpart in its properties you will find the list path to which it is storing data) and then you can generate monthly report from that list

i think you are barking at the wrong tree, this forum is for infopath, not for sharepoint designer, you better ask this question in some sharepoint designer forum

after every publish, the original template gets updated. the template is located at
your form library path\forms\template.xsn
to navigate to the template.xsn
open your document library -> go to actions -> open in windows explorer , forms

i think you have a fairly good idea of what you want to implement.
i would recommend following steps
1) generate the text file in infopath form itself. place all the contents in a single field and promote it.
2) create a utility that reads this field (use spobjects) and stores it somewhere. the utitlity will need to run ...
