You can't add a filter to the target of a rule in InfoPath out of the box. Perhaps try this? http://www.infopathdev.com/blogs/hilary/archive/2009/06/07/hack-manifest-xsf-to-filter-the-target-of-a-rule.aspx

Yuck. So, if you save your form as source files (from the publish menu) you could examine the manifest.xsf file in a text editor. Look for the xsf:view element for the Competencies' Appraisal view and see if you can find something that looks out of place. Only think I can think of is there are usually query params in a browser form that you can access via code that are not present when opened in the filler... but I can't think of anything out of the box that would tell you this, I'm afraid. I think the approach you have taken may be the best option... Maybe put a BIG expression box on the ...

I don't see a way to actually delete Attachments from an InfoPath List Form template - see attached screenshot. I'm not sure how you deleted it completely - can I see your form? You can attach it under the Options tab in a reply.

Got it. Any message? Apparently you can use PowerShell (SharePoint Management shell) on the server to track this down. https://blog.netnerds.net/2011/09/sharepoint-2010-powershell-delete-site-content-type/
