You might be able to add a validation rule that checks the filename and fails if it doesn't match your criteria. qRules lets you access attachment name info so you could just set a rule on the field that checks and removes it if the file doesn't have the right extension. For offline forms, Tom's link to Formotus is the only option right now. We've worked with them before and they have a good product.

Actually, there is one technique. You can right click the control, select Text Box Properties and choose hte Display tab. You will see a checkbox to Limit text box to 1 characters. When checked, you can also check the box underneath it to "Move to the next control automatically when the limit is reached".
