I'd sugggest that you check the Rules in the properties of the submit button, make sure its data sources/connections are working.
To approve the UDC file, go to your data connection library and find the UDC. The UDC name should display a down arrow, click on that and select Approve/Reject. The Approve/Reject window will show, so click ...

Well, I'm running out of options. But you can check with your SharePoint Administrator if your site is correctly configured to run InfoPath Forms Services. It should be enabled to render browser-enabled form templates. If that option is disabled, users cannot use a web browser to fill out the browser-enabled form and must use InfoPath Filler ...

Hi azandbergen,
Sorry to hear about your issue. When you click submit while previewing the form as an application, does it work? And did you already approve the UDC file in SharePoint?

In Form Options > Compatibility, did you enter the address to your SharePoint server? It's right below the dropdown where you selected "Web Browser Form".

Hi George,
I don't understand why the columns are removed. I suggest to check if there are any errors shown in the Design Checker, go to File > Info > Design Checker. See if there's any data connection errors shown after saving the form to InfoPath 2007 version.

Hi Ben,
If the same values are being used repeatedly, try adding those as default values to your fields. You can also pre-populate your form with data using secondary data sources and controls such as dropdowns to help the users quickly fill out the form.

Can you try adding a rule in your dropdown that sets your Results field with a value like ../my:ThresholdA * xdXDocument:GetDOM("Thresholds")/dfs:myFields/dfs:dataFields/d:SharePointListItem_RW/d:Low (where ThresholdA is your text field for entering "A" thresholds), add a condition if Modifier = "Low". Then ...

Hi dw999,
May I ask how do you identify if a number entered belongs to Threshold A, B, or C? For example, does 1-10 belongs to A? And what's greater than 10 belongs to B and so on..?

Hi George,
In your form library, did you change the settings to open the form template in the browser instead of client? Also, since those staff accessing this form are running Office 2007, why don't you try saving your form as InfoPath 2007 Web Browser Form Template or InfoPath 2007 Filler Form template and then re-publish it to your site?
