I created a Rooms Reservation Form which has Date / Time and there is also a SharePoint List of Rooms. As new items are added the calendar list stores the Room, Date / Time, and several other fields.

You have two different sites? One is named 2012 and the other is named 2013? Is Activities the name of the form library? If you are no longer getting the message about replacing the template (form conflict) I guess I'd assume that since the URL is different IP is keeping them straight some how - but I hadn't realized that IP was going ...

Thanks for your response and the sample form which I understand.
Editing the embedded datasource file I add my own xml data which worked fine.
As my forms are browser my ideas to add my own store number and names to the xml would not work.
Instead of using the xml I tried with a sharepoint list with 2 columns.
