Can you please attach your template and explain what the main issue is? thanks!

Add a field or attribute (say field1) with a default value to 0, and in the submit button add a rule to set a fields value (field1 = sum(field1) + 1) - this way it increments field1 value. Concat this field in your subject field.
Hope that helps!

Use concat function - attached a snapshot of how it looks. Attach your template if you sill run into issues.

When you create your submt data connection, you will get a dialog that will give you a chance to specify the name of the file - for this, you can use values from the fields in the form.

1. Go to the data source task pane and double click on drop down field.
2. Select Fx button for default value.
3. Select Insert field or group.
4. Select your secondary data source, and select value.
5. Preview the form.
Hope that helps!

You can use filter condition to filter your data.
Attached a sample template with two drop downs, and added a SQL data connection.
Let me know if you run into issues.
Good Luck!

Attached a sample form that contains couple of controls (made them read-only after a form is submitted) and a section with checkboxes (made it visible after a form is submitted).
Look at the conditional formatting for each one of them to have an idea....
Hope that helps!
