Scenario QA Form, have to work out a percentage final score based on the 16 answers responses
16 questions with Yes No and some N/A as answers.
No is fine it does not add to the score. YES and N/A would adjust the score percentage

Sorry for not being clear. Yes it's a browser form. But how would I write code using Sharepoint object model? I would like the code to submit data and infopath attachment to sharepoint list.

Check the value of the queryxml immediately before the query executes. It seems like the value in queryxml might be invalid or badly formatted (or even blank).
