 Ernesto,  Appears something as simple as that `Always' check box was the key. Not sure how that was not enabled but it certainly did the trick.  Thanks for seeing and answering so quickly!  

I would check the Postback settings of the button that executes the qRules command. Make sure this button is set to Always. I would also place the Result, Error and Success nodes (from the QdabraRules data connection) on the canvas, to see if we can obtain any additional information.

I have generated a number of these and normally once it works in preview I am set. This one, however, is having problematic issues attaching the file. In preview it works fine... attach, press button and link is generated. Once I push it to a Form Library on SharePoint (publish, add the form to site collections and attach it to the form library ) ...

So - you'll have to get the file from the file system (form will have to be full trust) and encode as Base64. Then set the pic control to the encoded Base64 string.
