Never mind i got the solution Instead of using GetCommonMembership i used GetUserMembership DC and it worked. Thanks Rohan.

Thank you Hilary.  I attached the form on my original post.  I'm sure you know, but you may have to change the respond to email address.  Right now, it is coming to my email address.  Appreciate the help so much.
