when u publish a code less form, it is published to a particular doc lib and the template resides in the doc lib itself, but when u publish a template having managed code the template is saved at
http://your site collection/FormServerTemplates/yourtemplate.xsn
so that might be causing the problem, the anonymous users wont be havign access to ...

well the messges dont have attachment controls, and i dont know how u will be able to access the messages from your computer that does not have internet connection

it is possible to change the default placeholder for picture control. by editing the appropriate view.xsl file and editing the appropriate tag.
But i am not sure if we can do any thing for file attachment control.

you are exactly right. you will only be able to retrieve those records on which you have atleast read permission.

if you have implemented item level permissions on the sharepoint list, then it is ok, the dataconnection will fetch only the records for which the user has access. So just create a secondary data connection to that list and fetch the column(s) and bind it to the dropdownlist..
