The problem with that approach is that I am trying to submit into a database table, meaning I am unable to add the "count" and "lastitem" fields .. which are needed :( Unless there's a way to submit user created fields into the database table?

If the table is not populating, then the sAMAccountName is coming from somewhere else, perhaps another rule or data connection. Can you send us the form? Thanks.

It does not appear to be retrieving any information at all. Not sure how it's getting the sAMAccountName. Please see attached screen shot.
