Open your share point list view and go to modify this view. In that you will find Filter option, in that select your managers column(For this you have to promote this column) name, select equal to in next drop down and give the name of the manager whose forms you want to show in the list. I think it solves your problem.

Add a reference field to schema, set this field value to 1 if you click on full details button and set to 0 if you click on new form button. Populate the fields with query values if the reference field value is 1 otherwise make the fields blank. OR You can add another view and display this view when you click on New Form button.
