If you make columns in your document library required the user will have to fill them out when they upload a file.
-
Do you just need to display the data? If so you could use calculated values instead of text boxes, and then under the properties, set the decimal places... You may also be able to set this formatting option on the text box if the field is a decimal type - right click the text box, and under the properties, if it is a type that has formatting ...

Can you create a sample form that duplicates the logic you are trying and isn't connected to anything on your network with made up questions? I just need a sample to understand your questions - otherwise I'm going to guess, and I never guess right. :-)

See attached. Save the file locally, right click and select design. Then preview. There is one table with a bunch of years filled out and the same table below with conditional formatting rules on it.

Any chance you can attach a copy of your form under the Options tab in a reply? It would help me to see it. :)
-
Try this one - Are both of these dropdowns using the same list? It looks like that from the filter, like the same list item would provide data to both? I also added a text box bound to the subnet mask field so you could see the value being set, in case that helps figure this out.

If you know the name of the form, you can use a default value for the form link. It looks like you have a formname field you are using in your submit, so just add another hidden field for the form link, copy out the URL of an opened form, and concat in the form name. Since this is a browser form, you can switch views and send an email (in Filler, ...
