I've realized that the problem actually occurs when you edit the properties, and change the name there.  It doesn't create a duplicate when you edit the name, but when someone goes to open edit the actual form again, it creates a duplicate.  Sorry for the misinformation.  Does anyone know why it does this?

I'm not sure if I fully understand.. I am not able to set any other rules than an action. I am able to bind the data to a table, and query for results. However to do what I am looking to do would be a good number of queries along with connections.
