Probably just a normal InfoPath form then - you can publish it to a SharePoint form library for users to fill out. Under the form options, you can change the compatibility to a browser form (or you can use File > Save As and save as browser form)
I'm going to sound really out my depth here ...

So - you could query the library, get a count of the forms, and increment it by one, then zero pad it get the next number. The reason I suggest doing this when you submit is to prevent two or more users getting the same number.

No - because you don't have a way to know how many times the template has been opened. You don't know how many filled out forms have been saved.
You'd need to have a way to find out what the number should change to - have 100 forms been filled out? 5? 21? The form logic needs to be able to find that out and set the number ...

Hi!
qRules will submit the mappings in the order they are in the mapping file.
So - you can map whatever list you wish first, so that the SharePoint IDs are generated and populated, then map those ID fields for the other list(s).
The forum kinda blew away your formatting in your post, so I'm having a hard time understanding how these lists ...

How do I publish this to my list so that users will launch the form direct from SharePoint?
I'm lost, help!
Thanks!
