Is the action prior to setting the Approve field to true getting executed? Are all the actions failing? If this is a browser form and all the actions are failing it could be the post back issue.

Okay I figured out my first blunder: the hyperlink data sources were local to the form, I needed the source to be within each list so it would repeat as needed. Is it possible in Infopath to have a control "on-click button" to submit the form to associated Sharepoint List and then open a link? I've solved it. If anybody out there in internet-land is interested:
1. Select where on Infopath form you want the image displayed
2. Insert ‘image’ from controls panel
3. Right-click on the image control, and select ‘picture properties’
4. Select the icon next to the ‘XPath’ field
5. Select ‘show advanced view’
