Date fields in Infopath forms store their text as YYYY-MM-DD so you can use substring function to strip the first 4 characters.
As for the incrementing id to represent the count of previous appeals, unless you have a way of retrieving previous tally submissions made via the form (maybe via a secondary data connect? are you using SQL ...

You can format the field values directly into the view of the form, much like you do when you have the questions text and provide a field for the user to fill in, you instead have the summary text with the values of the field placed accordingly in the layout.
This doesn't require you to have extra fields or data structure to present the ...

Yep, modifying your form templates can sometimes be hit and miss for backwards compatibility with forms from the old versions.
Drastic data structure changes can break the previous forms.
Your approach is exactly what I have tried in the past and I have been met with varying success/no success. Sometimes I've had to 'pad' ...

Not sure if its the same issue, but Infopath forms viewed in Infopath for me always scroll to put the first selectable control at the 'top' of the window.
Try putting a 'dummy' button, very small at the top left of your view. When a view loads it will default focus to this control.

Using a sig tablet (have no experience) but maybe the sigTablet itself has a uniqueId you can retrieve from the attached device which you could use as the 'prefix identifier' in my suggestion below.
I usually end up going for a datetime stamp and prefixed with some other identifier, like the current logged in userId.
Other ideas I've ...

As a rule on the field - setting it's value.
Formula: concat(substring(translate(., "-", ""), 1, 2), "-", substring(translate(., "-", ""), 3))================================================================

This sounds like a question which might have been asked before.
But I decided not to search myself - but still try and give you an answer.
Maybe there are better methods already, or this could be one which hasn't been done.
This is a JScript method which I place behind a button.
//load the template.xml file into a new memory XML DOM
