Does someone fill this form out each time they create a presentation? Short answer is - yes, InfoPath would work well for this - but you could also just use a SharePoint list and use views for your various reports. Your data sounds like it is not very complex.... And how, in the form, are you setting your CAML fields? Are the fields on your form? Are you using code or rules to set the values?

Oh, that sounds right. What is the result you are getting when you try this? Is the dropdown set to use display name for both the display and the value? Maybe drag the data field that is currently bound to the dropdown onto the form again as just a text box so you can see if any value is getting set at all?

I'd guess this has to do with your submit connection - it probably only submits to the source library. You probably need to add another connection to the destination library and in your submit rules, you can determine which to submit to. Is the logic as simple as new forms submit to the source library, and edited forms submit to the ...
