I'm not 100% sure I understand what you are currently doing, but I think you need your Type option to be inside your repeating group -- it seems that each instance has a Type, and the Type needs to be selected -- is that the case?
If not, if, instead, Type applies to the form as a whole, you add rules to the repeating group itself with a ...

Does your stored procedure require any paramaters? Where does it get the data it is supposed to be inserting into the database?

After a file in attached in an image picker, the name is no longer present in the base 64 stored in qRules. If you attach with a file picker, you'll maintain the name. Due to this InfoPath limitation, all images attached via image picker end up named "Picture".

Where is your XML submitted? SharePoint Library? With SharePoint 2010, you can add a data connection to the library your form submits to and select to get back information about just the current item. That should get you the Title field, which should be your file name..... you'd need to use a concat formula to include the extension if you need ...
