but i need the in button when clicked to clear the other boxes

on the above form when someone clicks the in button i would like to have the boxes to the right clear automaticly

Please can some one help 

Not all of your images are showing but you should be able to add a rule to your radio button.

Set the condition for when it equals in.

then use set field actions to set the other fields to blank or no field.
