How can I add a repeated section of the form?

I have a form where I would where if the user checked the Add More Parts checkbox, another "section" would be added to the existing form. This added section would contain the fields circled in the attached picture. How can I do this?

Put the fields in the section, and hide it unless the Add More Parts field is equal to true?
