Strange Content shows up in Published Browser Form for LIST Item when set to launch dialog yes

I have an issue where a published list form is showing strange content from the SharePoint page (list view) where the user accesses the NEW form. The form opens and functions fine. However, as soon as data is refreshed on the form, the Content Steward and Page Publisher appear at the bottom of the form, crammed in together. It doesn't look good. I'm wondering if somehow the form is pulling in the footer from the SharePoint page and placing it at the bottom of the form.

Can anyone assist? I've been racking my brain over this.

I tried deleting connection files and fields I though might be causing the issue. So far, no luck.

     Update:

While trying to fix this, I noticed that when I set the Launch Forms in Dialog to NO, the page works fine. It's almost as if when I set the launch in dialog to "YES" I'm actually still getting all the same content you would see as if it was set to NO but now it's crammed into a dialog. All of this seemed to occur when I tinkered with my Receive connection to the SharePoint list where I modified it to add more values.

Looks like it might be pulling in a CSS for some reason (bottom-banner) as if it was not in a dialog.
