Hello, I need to create Form that displays

Name: (drop-down list)

ID (drop-down list)

People (people picker)

Initial date (date picker)/ End Date (date picker)/  Classroom (drop-down list)

And in this part to get a repeat box for the last 3 fields. The issue is that every time I create a new table or try to put the name or id or people it gets me a repeat box with that Field.

I want that name, id and people replicates in every date y create, but asking the user to input only 1 time. 

There are a few approaches you can take here - I'm attaching a sample form with two repeating tables. One displays the non repeating fields as calculated values. The other has fields in the repeating table that use defaults. Save the file locally, then right click and select design to open in design mode. From there, you can preview the form, fill out the first three fields, add rows to the tables, etc.
