Not sure which template to start off with

I have to create a form (more like a questionaire) that will be published to a Sharepoint Site. The users will be a mix of 2010 and 2007. I will be using Infopath 2010, but with all the templates I am a bit confused

as to which one to use.

Also, I have a question on how I would accomplish this..... Business people devised an excel sheet where they enter from 1 to X number of products in columns and the questions are row based

                                                                       Product 1              Product 2                Product 3               Product 4                   Product 5                Product 6  .......

    Question 1 about the product.                         Yes/No                   Yes/No                   Yes/no                  Yes/No                      Yes/No                    Yes/No

Question 2 about the product                          Low                        Low                          Med                      High                           Med                       Low      ....

    ......

They devised an excel sheet where they enter from 1 to X number of products in columns and the questions are row based. I would like to have them select the products once from a

drop down list box...I dont want to have them select the product each time they answer the question,. There could be 20 products selected and 40 questions approx.

I see there is a horizontal table...  Thanks

Also, they will select products from a drop down list box and there will be a number of questions regarding the products selected. For instance, if they select 12 products

    2007 or 2010 what?  SharePoint?

If all users have access to SP 2010 form library, I'd use that. Then put your form inside a webpart page and you should be good to go because users will use a browser to acccess and use the form.

You can use either a blank form or a Form library template. It makes no odd really (as far as I know) since you'll be manually publishing your form to X library and submiting your data to X library.

Do they select products first, then answer questions? Or is the table as in your diagram presented 1st.

If it is as in your diagram, you could a repeating table.

To my mind, since you could have a lot of products and questions, you might want to think about a view per product or split your table into sections, depending on products selected.

It all depends on what is selected first and how. If I select all 20 products and have 40 questions how is that going to look a laptop screen?

I've recently done a questionnaire/quiz, admittedly the questions all followed on from one to an other. I created a view for all questions to be stored, which contained everything that I could possible want in terms of questions and answers. Then each questions became a view. This allowed for easy scoring of answers and the ability to create a summary view after the quiz was completed. Also it made managing sections and hiding information much easier per view, than on one long list.
