I have an info path form that container a embeds hyperlink to a spreadsheet that must be filled out. It is required when each info path form is submitted via sharepoint... It opens it saves ... That's working great, however... Every time someone goes in and opens it to fill it out it saves the changes to each instance of the spreadsheet. In all the forms submitted... I need it to save a new copy to each form.

New form> is created when a new change request is raised
Once approved to proceed that same form ( and assessment worksheet embedded in form) must go around to several assessor a to be completed. Then when all assessments are in. Goes to CCB,

I noticed that if you add your assessment to 1 you are adding them to every change request.. How can I fix this???
