    Questionnaire form

I'd like to know if it's possible to create questionnaire form connected to database. I'd like all questions to appear on the form and users would then select "Yes/No" from the drop-down and submit data to database.

Since form is connected to database, I'm not able to pre-populate the questions. Has anyone designed a similar form?

     Appreciate any help.

I imagine you'd need a database with two separate tables - one that stores the questions which will be your "query" table, and one to submit answers to.

I think a web service would be useful for you. Qdabra's DBXL tool might be of help - it has a QueryDB web service that allows you to query data from SQL; you can even build complex filters if needed. It also has a submit web service that will submit your data back to SQL. You can download it here for a free trial to see if it suits your needs.
