Trouble in opening up the initiation form of a workflow "The form has been closed error"

I created a simple workflow exactly as described in the following article:

    http://weblog.vb-tech.com/nick/archive/2006/09/04/1753.aspx

The WF has 2 IP forms which are created and published in the workflow/deployment files/ features/ folder.  I changed feature.xml & workflow.xml with the appropriate ID, name description, form URNs,..  It build without any error; also when change the NODEPLOY to DEPLOY in the build events, it deploys it nicely without error.  I do see the folder with the name of the workflow under 12/Templates/Features.  In that workflow folder i see only the feature.xml and workflow.xml.   I don't see the IP forms!  Where are these forms supposed to be placed after the DEPLOY rebuild?

Then i went ahead to attach this WF to a  form library.  I clicked the Form settings --> Workflow settings --> Add a WF --> i see my WF in the drop down, choose that --> next, then i get the error from the forms services saying the form cannot be found, if i enter again it gives me the "The form has been closed" screen.  I am unclear about the following things:

    1. Do we publish the IP forms to the Visual studio 2005/ projects/workflow name/deployment files/features folder? or create a form folder in there(Visual studio 2005/ projects/workflow name/deployment files/features /forms) and publish there?

    2. When i click the IP forms from the workflow solution, the forms are not opening; it gives me the message "The forms with the domain level security....". Is that the normal thing?  If not what am i missing? i set the forms security level to Domain, enable the browser compatibility & publish without any alternate access path.

    3. After deployment where are these IP forms supposed to be placed?

    4. Do we need to publish the form to the sharepoint server as well?

Please clear my doubts and spread your knowledge :).  Ahy help would be appreciated.   I have to get this working ASAP. HELP!!.

PS: I have the screen shots of the error when tried to add WF in a sharepoint Forms library. I can email that if anyone wants to see the error.
